Single-incision Laparoscopic Appendectomy for Acute Appendicitis in Pregnancy—A Report of Two Cases—

妊婦の急性虫垂炎に対する腹腔鏡下虫垂切除術（以下，LA）の報告は，本邦でも散見されるが，単孔式手術で施行された報告例は少ない．妊娠中に急性虫垂炎を発症し，単孔式腹腔鏡下虫垂切除術（以下，TANKO-LA）を施行した2症例を経験した．症例1は23歳・妊娠23週，症例2は33歳・妊娠15週で，腹痛を主訴に当院へ搬送となった．いずれも急性虫垂炎と診断し，緊急でTANKO-LAを施行した．術後経過は良好で，2症例とも正期産で自然分娩であった．妊婦に対するLAは低侵襲であるが，子宮への干渉を避けるため，ポート配置を工夫する必要がある．子宮底が臍高まで上がってきていない時期であれば，臍部からのみでアプローチするTANKO-LAが良い適応であると考えられた．
